## Study Design & Scientific Leadership

### Getting the design right

The most consequential decisions in observational research happen at the design stage, before a single line of code is written. Headwater Science's methodologists work directly with sponsor teams to build studies that are analytically sound, scientifically credible, and structured to withstand the scrutiny of reviewers who didn't run them.

### Study Design Consulting

Collaborative protocol development for comparative, descriptive, and causal research questions, including target trial emulation frameworks, eligibility criteria alignment, time zero specification, outcome definition, and sensitivity analysis planning. Our methodologists engage at the point where standard analytic templates aren't sufficient.

### Complex Subpopulation Research

Many of the most important clinical and regulatory questions involve patient segments where population-level findings don't apply — rare disease populations, high-risk subgroups, patients with specific treatment histories, or cohorts defined by criteria that standard databases don't capture cleanly. ### Clean Room Committee

An independent committee of statisticians, epidemiologists, and clinicians that reviews and recommends protocol modifications without access to unblinded results. The committee works from aggregated or blinded data, so that every design recommendation is made without knowledge of its directional impact on findings. A complete audit trail documents all activities and recommendations, providing the kind of transparent, pre-specified governance that regulators and HTA reviewers increasingly expect to see.
